Agreement for Services Rendered “Reputation Management” Setup Package

This is a contract entered in to by Mediate Lawsuit (hereinafter referred to as “the Provider”) and The Client (hereinafter referred to as “the Client”) on this date.

The Client hereby engages the Provider to provide services described herein under “Easy Review Site” Setup Package.” The Provider hereby agrees to provide the Client with such services in exchange for consideration described herein under “Payment for Services Rendered.”

Easy Review Site Setup Package

Includes the following:

Personalized landing page with detailed company information
Accessibility by clients/customers to leave feedback or reviews
Access to “respond to reviews”
Branded Email to initiate Feedback Request from clients/customers
Full Training to maximize use
One time charge to create landing page
Recurring maintenance fee covers User License and Daily/Weekly Processing of company data by Mediate Lawsuit’s programmers
Month to month terms, with no term contract (30 day “out clause”)
